구급대원이 병원을 결정하게 하자는 발상, 응급의료 이론에 대한 무지의 산물

[메디게이트뉴스] 최근 모 의원이 발의한 이른바 ‘응급실 뺑뺑이 방지법’은 환자를 이송할 병원을 구급대원이 지정하고, 응급실 측의 동의 없이 환자를 내려놔도 된다는 내용을 담고 있다. 정치권이 이를 지지한다는 소식이 들리니 참으로 기가 막힐 노릇이다. 이 법안의 핵심은 ‘응급실 선택권’을 구급대원에게 주자는 것이다. 그러나 응급의료체계의 기본원리를 이해했다면 이런 법은 결코 나올 수 없다. 응급의료체계(Emergency Medical Service System, EMSS)의 시원은 외상체계(Trauma Care System)다. 외상체계 이론의 기원은 세계 최고 미국 메릴랜드 외상센터의 창설자인 닥터 카울리이며, 구급대원이 중증외상 환자를 부적절한 병원에 내려놓고 떠나는 문제를 해결하기 위해 만들어졌다. 엉뚱한 응급실에 진입하면 진료절차로 인해 귀중한 시간을 낭비하게 된다. 환자를 가까운 병원에 아무 데나 데려다주는 것이 아니라, 시간이 조금 더 걸리더라도 수술 준비가 된 고급 외상 2025.11.11

의료붕괴 대책은? 우리나라 의료를 지속가능하게 하려면?

[메디게이트뉴스] 의료재앙이 일어난 발화지점은 전공의들이였다. 작년 2월 의대증원을 계기로 촉발된 전공의 사직, 학생 휴학, 의료붕괴, 윤석열정권의 계엄선포와 탄핵, 정권교체, 의대증원 철회, 그 후 올 9월 학생들과 전공의들이 복귀하면서 사태가 일단락 되는 듯 보인다. 그런데 달라진 것은 없고 상황이 더 오히려 더 나빠지고 있다. 한가지 확실한 것은 대한민국 의료는 2024년 2월 이전으로 절대로 돌아가지 못한다는 사실, 그리고 큰 희생을 치른 젊은 의사, 학생, 전공의들이 의료계의 암울한 현실에 눈을 뜨게 됐다는 것이다. 결국 이 사태로 인하여 국민, 의사, 특히 전공의와 학생들 모두가 루저가 됐다. 정치인들은 오로지 자신들의 기득권 지키기에만 몰두하며 의료계의 근본적인 문제는 그대로 두고 지엽적인 문제들만 건드리고 있다. 이들은 반성은커녕 하루가 멀다 하고 필수의료, 지역의료를 말살시키고 응급실 뺑뺑이를 더욱 악화시키는 최악의 법안들을 쏟아내고 있다. 국민은 병에 걸릴 경우 제 2025.11.11
