Isotretinoin의 경구투여 시 3.2 시간 내에 혈중 내 Peak level에 도달하고 경구 투여한 25%가 Bioavailability를 나타낸다. 기름이 많은 음식과 함께 복용 시에 Isotretinoin의 흡수가 증가되고 섭취해서 흡수된 Isotretinoin의 99%는 Plasma albumin과 결합한다. 간에서 4-oxo-isotretinoin으로 대사된다.

Isotretinoin은 다른 retinoic acid와는 달리 Retinoid receptor와 결합하지 않는다. Isotretinoin의 다른 메커니즘으로 예측한 내용은 다음과 같다.

로아큐탄® 복용 후 2주 정도가 되면 용량 의존적으로 sebum suppression이 일어나며 Kg당 1mg 복용 시 3개월 정도 복용하면 전체 피지의 90%가 생성이 감소된다.
Dose dependant : 30% with 0.1 mg/kg
40% with 0.5 mg/kg
70% with 1.0 mg/kg
90% with 1.0 mg/kg for 3 month
그러나 로아큐탄® 사용 중단 시 2~3주 내에 Sebum 생성이 증가 되고 10~12개월 정도 되면 거의 정상으로 돌아온다.
[표1] 0.1mg을 사용 하는 것보다 0.5mg나 1mg을 사용시 Sebum 억제가 확실하게 일어나고 사용 중단 시 2~3주에 다시 Sebum의 생성이 증가되고 점점 정상치료 회복됨을 알 수 있다.
[그림1] 정상적인 Sebaceous gland에 12주 동안 Isotretinoin을 투여 시 Sebaceous gland가 소실 되는 것을 볼 수 있다.
Isotretinoin은 새로운 면포형성을 방지하게 된다.

작용기전
- comeocyte를 연결하는 comeosomes의 형성을 억제 -Alteration of intercellular lipids (ceramides 나 sphingolipids의 생성을 감소 시킴으로써 comeosomes의 분해를 촉진시킨다.)
[그림2] Isotretinoin 사용후 각각 사용전,3주,7주,12주 사용시의 comeocyte layers의 변화로 comeocyte layers의 층이 감소되는 것을 확인할 수 있다.

Isotretinoin이 자체적으로 bactericidal 이나 bacteriostatic 효과는 없다. 그러나 Sebum의 억제효과로 인한 2차적인 효과를 나타내고 있다. -> Decrease of nutrition for P.acnes, even Gram negative bacteria
장기간 고용량 환자의 경우 Side effect로 Colonization of Staphylococcus aureus 의 side effect가 나타나는데 이런경우 mupirocin 이나 fusidic acid oint를 도포하고 있다.
